Proper wording for wedding response cards?

I need to include menu choices on my wedding response cards. Is there a formal way to word it so that guests know to write their names next to their choices? I need to know which people on each card are selecting each of the entrees. For example, if Mr. and Mrs. Smith are both attending but they just check one chicken and one beef, I won’t know which is which. But I think it sounds odd to just say "Please write your name next to your entree choice." Also, can I write just write "chicken" and "beef" or do I need to include the exact entree title? I hope this makes sense. Thanks!

    Do not write chicken or beef on the response card, word it exactly the way that the chef does. Why do you need to know who ordered what? Your chef does not need to know that Mr. Smith wants Chicken and Mrs. Smith wants beef. Just do it like this

    ____ Rosemary Chicken

    ____ Filet Mignon

    Example: If you have invited Mr. and Mrs. Smith and Family they will write 3 Filet Mignon and 1 Chicken

    We actually didn’t have them put their dinner choice with their name. The waiters bringing out the food knew how many choices of what were at the table then just asked who had the chicken and who had the strip. It worked out fine. If you would really like to include it you would probably need a large response card and put something like

    Mrs. Jones is attending ___ is not attending ___
    chicken ___ beef ___

    Mr. Jones is attending ___ is not attending ___
    chicken ___ beef

    Do it like a little test or poll. Head it "entree choice" then put "beef" and "chicken"
    Under that write Mr. Smith. A box for him to check under beef, one under chicken.
    Then Mrs. Smith. Same thing.
    Every named person checks a box under the entree she chooses.

    It helps to number the reply card as sometimes you cannot read their writing.

    The waiter isn’t going to know Mr/Mrs. Smith so I think just putting

    I would prefer

    Chicken________________

    Beef__________________
